Family membership covers all residents in the household, including children, parents, relatives, significant others, and all permenant residents of the household. Q. How much more do the Pharmacy and Vision memberships cost? A. The Prescription Drug and Vision Care benefits are absolutely FREE with the Dental Plan membership.   Q. Are pre-existing conditions covered? A. Since the dental plan is not insurance or a health care organization, all pre-existing conditions are covered except orthodontic treatment in progress. Q. How Does Your Dental Care Plan Work! A. Our Dental Care is a dental referal organization that offers memberships in its discounted dental plan. Members can save 25% to 65% on all restorative and cosmetic work (fillings, crowns, braces, etc.) and up to 80% on preventative work (teeth cleaning, x-rays. etc.) performed by a general dentist. Specialist's fees are discounted 25%.   Q. How do I pay my MEMBERSHIP fee? A. Monthly or quartery payments are made ONLY by Electronic bank draft system or by credit card. INVOICING IS DONE ONLY FOR ANNUAL MEMBERSHIPS PAID ONE YEAR IN ADVANCE. Indivual membership is $9 per month and family membership is $15 per month. Q. How do I use the Plan? A. Within 10-14 working days of your enrollment, you will receive a Membership Information Guide, Membership Identification Cards and a Dental, Pharmacy, and Vision Care Directory. Present your Identification Card to any of our providers and you will be elegable to get the discounted fees. You pay the provider at the time of service.   Q. Who are the Dental, Pharmacy, and Prescription Care providers? A. Our dental program has assembled one of the largest networks of dental providers in the nation, and we are continually adding to this base. These dentists are well established, with successful dental practices. Our Prescription Card may be used at over 50,000 pharmacies nationwide, as well as through the mail order. The Vision benefits are available nationwide at more than 12,000 providers. Q. Are there any deductibles or claim forms? A. There are no deductibles, no claim forms to fill out, and no limits on visits to your dentist.   Q. Do I have to wait to see a dentist A.
